Job summary

Innovate Rehab and Wellness, a leading skilled health care provider, is seeking a Licensed Certified Occupational Therapy Assistant (COTA) to join a SNF team in St. Louis, MO.

The role supports delivery of high-quality OT services, helping residents regain independence and improve quality of life under OT supervision. The position is full-time with a Monday–Friday schedule and a competitive hourly rate based on experience.

Qualifications

• Associate’s degree from an accredited OTA program.
• Current NBCOT certification.
• Valid state license as an Occupational Therapy Assistant.
• SNF or long-term care experience preferred.
• Strong communication and interpersonal skills.
• Able to work independently and as part of a team.
• Committed to patient-centered care and rehab excellence.

Responsibilities

• Implement treatment plans created by the Occupational Therapist
• Provide direct patient care per state and facility guidelines
• Monitor and document resident progress; report changes to the OT
• Educate residents and caregivers on adaptive techniques and equipment
• Collaborate with nursing, PT, SLP, and other healthcare staff
• Maintain accurate, timely documentation in the EMR
• Participate in care planning and discharge planning
• Ensure compliance with HIPAA, CMS, and all regulations
