Job summary

Innovate-Rehab-and-Wellnes seeks a Licensed PRN Certified Occupational Therapy Assistant to join our Skilled Nursing Facility team.

You will deliver OT services under supervision and help residents regain independence through individualized interventions. The role emphasizes one-on-one therapy and flexible PRN shifts within a supportive, therapist- and veteran-owned culture.

Qualifications

• Associate degree from an accredited OTA program.
• Current NBCOT certification.
• Valid state OTA license.
• SNF or long-term care experience preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OTE.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other staff; maintain EMR documentation.
• Participate in care and discharge planning; ensure HIPAA compliance.
