Job summary

Innovate Rehab and Wellness in Missouri is seeking a Licensed PRN Certified Occupational Therapy Assistant (COTA) to support a Skilled Nursing Facility. You will provide high-quality OT services and help residents regain independence through person-centered interventions.

This PRN role offers flexible shifts, a focus on one-on-one care, and opportunities for growth within a therapist- and veteran-owned company culture, while collaborating with the care team.

Qualifications

• Associate’s degree from an accredited OTA program.
• Current NBCOT certification.
• Valid state license as an Occupational Therapy Assistant.
• SNF or long-term care experience preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Implement treatment plans created by the OT.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
