Job summary

Responsibilities

• Conducts assessments and reassessments of patient function using diagnostic procedures.
• Plans, implements, and supervises individualized therapeutic programs to improve physical and sensory functions and independence.
• Coordinates with Rehab Director to integrate therapy into the overall plan of care.
• Instructs patient in therapeutic, compensatory, and self-care activities to improve independence.
• Observes and documents patient responses to treatment and changes in condition.
• Educates family members and other staff to safely support therapy.
• Participates in patient case conferences and QA/QI activities.
