Job summary

• Conducts assessments and reassessments of the patient’s level of function.
• Plans, implements, and supervises an individualized therapeutic program to restore or improve function.
• Coordinates with Rehab Director to integrate the program into the plan of care.
• Instructs patients in therapeutic, compensatory, and self-care activities.
• Observes and reports patient responses and changes in condition.
• Instructs other agency personnel and family members to assist safely.
• Participates in patient case conferences to coordinate the plan of care.
• Participates in QA and Continuous Improvement activities.
