Job summary

Responsibilities

• Conducts assessments and reassessments of the patient’s level of function by applying diagnostic and prognostic procedures.
• Plans, implements, and supervises an individualized therapeutic program designed to restore physical function, sensory-integrative functions, or greater independence in activities of daily living.
• Coordinates and collaborates with the Rehab Director, or designated professional, to ensure integration of the therapeutic program in the plan of care.
• Guides and teaches the patient in the use of therapeutic, compensatory, creative, and self-care activities for improving function and increasing independence in daily living.
• Observes, records, and reports the patient’s reaction to treatment and any changes in condition or treatment; documents involvement in decisions affecting care.
• Instructs other agency personnel and patients' family members in phases of occupational therapy to safely assist the patient.
• Participates in patient case conferences to coordinate the plan of care and promote positive therapeutic outcomes.
• Participates in continuous assessment and evaluation of the agency via Quarterly Record Review or quality assurance/improvement programs.
