Continuum Therapy Partners in Monroe, Wisconsin, is seeking an Occupational Therapist to join our team on a PRN daytime schedule at Monroe Manor Nursing & Rehab Center. The role focuses on delivering person-centered OT services and collaborating with interdisciplinary teams.

You will evaluate and treat patients to improve functional independence, document progress, and educate patients and families on adaptive equipment and home safety.

Formación

• Master's or Doctorate Degree in Occupational Therapy.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsabilidades

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required

• Current, unencumbered state Occupational Therapy License; NBCOT Certification required

• 1 year of previous experience as an Occupational Therapist preferred
