Job summary

Continuum Therapy Partners is seeking an Occupational Therapist at Heritage Hall Wise, Wise, VA. The role focuses on evaluating patients, developing individualized treatment plans, and improving functional independence in a PRN daytime schedule.

The ideal candidate will hold an OT degree, an active state license, NBCOT certification, and ideally one year of experience working with diverse patient populations in outpatient or facility settings.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required
