Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist for a PRN daytime schedule at Heritage Hall Big Stone Gap, VA. The role focuses on delivering individualized OT care and improving patients’ functional independence within a supportive team.

Qualified candidates hold a Master’s or Doctorate in OT with an active state license and NBCOT certification. Prior OT experience is preferred to contribute to planning and patient education.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.
• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
