Continuum Therapy Partners, LLC is seeking an Occupational Therapist to join our patient-centered team. We hire with a therapist-led philosophy and support individualized treatment plans to improve functional independence.

Requirements include a Master's or Doctorate in Occupational Therapy, MN state license, NBCOT certification, and approximately one year of experience preferred. PRN schedule at Heritage Manor in Chisholm, MN; contact Maggie Sheffield.

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients’ functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
