Job summary

UW Health in northern Illinois seeks an Occupational Therapist for its Home Health team in Rockford. This 100% FTE role requires an OT degree and Illinois licensure, with a valid driver's license and CPR/BLS. You will provide therapy services at the 2550 Charles St.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current license from the State of Illinois or in the process of acquiring licensure.
• Must possess a current, valid driver's license.
• Basic Life Support/CPR Certification.
• An applicant for employment as a physical, occupational, or speech therapists must present acceptable evidence of licensure or authorization to test at the time of employment.

Work Schedule:

100% FTE, Sunday-Saturday 8:00am-5:00pm, you will work at UW Health Home Health 2550 Charles St.

Additional components of compensation may include:

• Evening, night, and weekend shift differential

Qualifications
