Job summary

UW Health in Rockford, IL is hiring an Occupational Therapist for a full-time, 100% FTE position. The role entails providing rehabilitation services at the UW Health 209 9th St Clinic, with day shift hours and rotation.

You will contribute to patient care through evaluation, treatment planning, and collaborative practice within a multidisciplinary team. Qualifications include graduation from an accredited OT program, ongoing CEUs, and active Illinois licensure with BLS/CPR certification.

• Participates in ongoing CEUs and education.
• Current Illinois state license and BLS/CPR certification.

Job description

Work Schedule
100% FTE, full-time. Day shift, Monday -Friday 8-4:30, one weekend day (Sat or Sun) every 4 weeks, one rotating holiday per year. You will work at the UW Health 209 9th St Clinic.

Additional components of compensation may include:

Evening, night, and weekend shift differential
Overtime
On-call pay

Qualifications

• Graduate of an accredited Occupational Therapy program. Required
• Participates in educational programs on an ongoing basis and completes mandatory CEUs. Required

Licenses & Certifications

• Current license from the State of Illinois per department policy. An applicant for employment as an occupational therapist must present acceptable evidence of licensure or authorization to test at the time of employment. An applicant who fails to provide this documentation will not be hired. A new employee will not be allowed to work as an occupational therapist until evidence of licensure eligibility is provided. Required
• Basic Life Support/CPR Certification. Required
