Job summary

Northshore Home Health Care in Charlottesville, VA is seeking a dedicated Occupational Therapist to provide personalized in-home and senior-living community care using the Geriatric House Calls model. You will assess mobility, develop care plans, and deliver evidence-based therapy to support independence and safety.

Collaborate with an interdisciplinary team, document progress in EMR, educate patients and caregivers, and incorporate telehealth to extend services.

Qualifications

• Valid Occupational Therapy license.
• OT degree from an accredited program.
• Proficiency with computers and EMR systems.
• Current CPR certification.

Responsibilities
