Job summary

Aura Health in Maryland is seeking an Occupational Therapist to deliver in-home, one-on-one care to older adults in private residences and senior living communities.

You will evaluate, develop and implement personalized treatment plans to improve mobility, independence, and quality of life, while documenting progress in the EMR and collaborating with PTs, SLPs, and fitness specialists. Telehealth may be used to extend access and flexibility.

Qualifications

• Active OT license or eligibility in the state of practice.
• Graduation from an accredited OT program.
• Proficiency with basic computer use and EMR systems.
• Current CPR certification.

Responsibilities

• Provide personalized occupational therapy in home and community settings.
• Develop and implement individualized care plans.
• Support patient wellness, mobility and independence with evidence-based treatments.
• Collaborate with interprofessional team and educate stakeholders.
• Support delivery of virtual care when appropriate.

Job description

Deliver one-on-one outpatient care to older adults in private residences and senior living communities using the Geriatric House Calls™ model.

Perform evaluations, develop personalized treatment plans, and provide evidence-based therapy to improve mobility, independence, and quality of life.

Document patient progress accurately within the EMR system and collaborate with an interdisciplinary team including PTs, SLPs, and fitness specialists.

Educate patients, caregivers, and staff to promote safety and aging-in-place strategies.

Utilize telehealth and virtual care options to enhance service accessibility and flexibility.

Qualifications & Skills

• Valid OT license or eligibility in the practice state


• Degree from an accredited OT program


• Proficiency in basic computer and EMR systems


• Current CPR certification
