Job summary

Preferred Home Health Care & Nursing Services is seeking an occupational therapist to deliver one-on-one care in home and community settings under the Geriatric House Calls model. You will evaluate needs, create individualized care plans, and provide evidence-based treatments to improve mobility and independence.

You will collaborate with PTs, SLPs, and fitness specialists, educate patients and caregivers on aging-in-place strategies, and use telehealth to increase access while maintaining

• Valid OT license required.
• CPR certification required.

Job description

Deliver personalized, one-on-one occupational therapy to older adults in private homes or senior living communities, enhancing mobility, independence, and quality of life using the Geriatric House Calls™ model.

Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists, and educate patients, caregivers, and staff on safety and aging-in-place strategies.

Maintain accurate documentation within EMR systems and adhere to regulatory standards.

Ideal candidates hold a valid OT license, have basic computer proficiency, CPR certification, and meet physical demands such as lifting and patient assistance.
