Job summary

DayToDay Health is seeking an experienced occupational therapist to provide one-on-one care to older adults in private homes, senior living communities, and community settings. You will evaluate needs, develop personalized treatment plans, and deliver therapy sessions to promote mobility and independence.

You will document progress in EMR, collaborate with an interdisciplinary team of PTs, SLPs, and fitness specialists, and educate patients and caregivers to support aging in place.

Qualifications

• Valid OT license or eligibility in practice state(s).
• Proficiency with computers and EMR systems.
• CPR certification current.

Responsibilities

• Provide one-on-one occupational therapy to older adults in private homes, senior living, and community settings.
• Perform evaluations and develop personalized care plans, delivering therapy sessions.
• Document patient progress accurately in electronic medical records and collaborate with an interdisciplinary team including PTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place; incorporate telehealth when appropriate to increase flexibility and access.
