Job summary

Qualifications

Seeking licensed Occupational Therapists to provide personalized, one-on-one care to older adults in private homes or senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatments.

Responsibilities include conducting evaluations, developing care plans, delivering therapy sessions, documenting in EMR, and collaborating with interdisciplinary teams of PTs, SLPs, and fitness specialists. Educating patients, caregivers, and staff to promote safety and aging in place, with optional telehealth/virtual care to enhance flexibility and access.
