Job summary

Health Wealth Safe is seeking an Occupational Therapist to deliver personalized, one-on-one home-based care to older adults under the Geriatric House Calls model. You will evaluate, create and implement individualized plans, and monitor progress in patients' homes or senior living settings.

The role emphasizes collaboration with a multidisciplinary team, focus on aging in place, and use of telehealth to expand access. A valid OT license, CPR certification, and EMR proficiency are required.

Qualifications

• Valid Occupational Therapy license.
• Proficiency with computers and EMR systems.
• Current CPR certification.
• Ability to perform lifting, repositioning, and transfers.

Responsibilities

• Conduct evaluations and develop individualized care plans for older adults.
• Administer evidence-based therapy to improve mobility and independence.

• Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists.
