Job summary

Affinity Home Health Care Inc. is seeking an Occupational Therapist to provide in-home and community-based therapy for older adults, focusing on enhancing mobility, independence, and quality of life through evidence-based treatments.

You will conduct comprehensive evaluations, create individualized plans, and deliver therapy while documenting progress in EMR. Collaboration with an interdisciplinary team and occasional telehealth will be part of care delivery.

Qualifications

• Valid OT license or eligibility to practice.
• Degree from an accredited OT program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet physical demands including lifting, transfers, and driving.
