Job summary

Vetted Solutions is seeking an Occupational Therapist to provide personalized, one-on-one care to older adults in private residences or Senior Living Communities. You will support mobility, independence, and quality of life through evidence-based treatment, collaborating with an interdisciplinary team and using telehealth to extend access.

Qualified candidates have OT degree, state license or eligibility, CPR, and EMR proficiency; driving to visits and PPE may be required.

Qualifications

• Valid OT license or eligibility in the practice state.
• Degree from an accredited OT program.
• Proficiency in computer use and EMR systems.
• Current CPR certification.

Responsibilities

• Conduct evaluations and develop individualized care plans; deliver therapy sessions to promote independence.
• Document patient progress in EMR accurately, adhering to regulatory standards.
• Collaborate with PTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place.

Role Overview:

An Occupational Therapist providing personalized, one-on-one care to older adults in private residences or Senior Living Communities, supporting mobility, independence, and quality of life through evidence-based treatment within our Geriatric House Calls™ model.

Key Responsibilities

• Conduct evaluations, develop individualized care plans, and deliver therapy sessions to promote wellness and functional independence.
• Document patient progress accurately in the EMR system, adhering to regulatory standards.
• Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Incorporate telehealth and virtual care to enhance access and flexibility.

Qualifications & Skills

• Valid Occupational Therapy license or eligibility in the practice state.
• Degree from an accredited OT program.
• Proficiency in computer use and EMR systems.
• Current CPR certification.

Physical & Additional Requirements

Ability to perform moderate physical effort, assist with patient transfers, and operate in home/community settings, including driving to appointments and wearing PPE when necessary.
