Job summary

RRC Solutions Group seeks a Hospital Liaison to lead post-acute intake and drive census growth in Brooklyn/Queens. This dual clinical sales and admissions leadership role interfaces with hospital case managers, discharge planners, social workers, and physicians to optimize transitions.

The role requires SNF/LTC experience, valid clinical credentials, and strong hospital networks; it offers autonomy, full-time on-site employment, and comprehensive benefits in NYC.

Qualifications

• Minimum of 2+ years of SNF/LTC liaison or admissions experience.
• Established hospital relationships in Brooklyn and Queens.
• Bachelor’s degree in Healthcare Administration, Marketing, Nursing, or related field.
• Active clinical credentials (RN, LPN, MSW, LCSW, PT, or OT) are a strong plus.
• Deep knowledge of hospital discharge workflows and SNF/LTC admission criteria.

Responsibilities

• Expand territory by engaging hospital case managers and discharge planners.
• Perform pre-admission evaluations and chart reviews.
• Convert clinical leads into admissions to meet census targets.
• Guide patients and families through sub-acute rehabilitation options.
• Collaborate with admissions teams to secure pre-authorizations and hand-offs.
• Monitor local healthcare trends and educate hospital teams.
