Cook Children’s Home Health seeks a Nursing Field Coordinator to support coordination of pediatric home health services in the Fort Worth area. The role involves providing in-field nursing care to align patient plans with clinical needs and regulatory requirements.

Responsibilities include documenting patient information for clinical and financial management and coordinating with the care team to ensure seamless service delivery. Pediatric experience is preferred, Texas RN licensure is required.

Qualifications

• Graduate of accredited school of nursing.
• 2-3 years' experience in hospital, critical care, or home health nursing.
• Pediatric experience preferred.

Summary

The Nursing Field Coordinator provides care in the field to assist in the coordination of services to meet the patients care needs. In addition, they document and provide information needed for clinical, regulatory, and financial management of the patient.

Education/Experience

Graduate of accredited school of nursing with 2-3 years' experience in hospital, critical care, or home health nursing. Pediatric experience preferred.

Certification/Licensure

Current Texas RN licensure. Current CPR certification. PALS Certification. Current Texas driver's license in good standing.
