United Future is seeking a Nurse Practitioner to provide primary care in the Greater Sacramento area. The role emphasizes serving underserved populations in a community health setting, collaborating with physicians and multidisciplinary teams, and maintaining comprehensive documentation.

The ideal candidate will hold an active California NP license, NP board certification, DEA and BLS, with 1–2 years of relevant experience and proficiency in EPIC EMR. Bilingual candidates are strongly preferred.

Qualificações

• Active California RN and NP licenses required.
• Current NP board certification.
• Current DEA and BLS certifications.
• 1–2 years clinical experience in community health preferred.
• Experience serving underserved populations preferred.
• EPIC EMR experience.
• Bilingual English plus Russian, Farsi, Pashto/Pushto, or Spanish preferred.

Responsabilidades

• Perform comprehensive patient evaluations with history and physical examinations.
• Diagnose and manage acute and chronic conditions.
• Order and interpret labs, imaging, and other diagnostics.
• Prescribe medications within scope of practice.
• Develop and implement treatment plans with Medical Director.
• Monitor progress and adjust care plans as needed.
• Initiate emergency procedures when indicated.
• Provide preventive care and health education.
• Maintain accurate documentation per standards.
• Collaborate with physicians and care teams to ensure high-quality care.

Comfortable 3 patients per hour
