Job summary

Acloché LLC is hiring a full-time Nurse Practitioner or Physician Assistant to deliver comprehensive primary care in the Greater Sacramento area. You will work with physicians and multidisciplinary teams to serve underserved populations.

Requirements include CA NP license, NP certification, DEA, and BLS. Relocation assistance is available. Schedule options include 8-5 or a 4x10 shift, Monday through Friday.

Qualifications

• Active California RN and NP license.
• Current NP board certification.
• Current DEA certification.
• Current BLS certification.
• 1-2 years in community health or underserved populations preferred.

Responsibilities

• Perform comprehensive patient evaluations including history and physical examinations.
• Diagnose and manage acute and chronic conditions.
• Order and interpret labs, imaging and diagnostics.
• Prescribe medications within scope of practice.
• Develop and implement treatment plans with Medical Director.
• Monitor progress and adjust care plans as needed.
• Initiate emergency procedures when indicated.
• Provide preventive care and patient education.
• Maintain accurate documentation.
• Collaborate with physicians and care teams.
