Job summary

ExcelliSearch, Inc. is seeking a full-time Nurse Practitioner or Physician Assistant to provide comprehensive primary care within a community health center network in the Greater Sacramento area.

Compensation ranges from $131,000 to $170,000 annually, with relocation assistance available and options for a traditional 5-day schedule (8–5) or a 4-day, 10-hour shift. This role emphasizes serving diverse, underserved populations in collaboration with physicians and multidisciplinary teams.

Qualifications

• Active California RN and NP license required.
• NP board certification required.
• DEA license required.
• BLS certification required.
• 1–2 years clinical experience preferred, community health/underserved populations advantageous.

Responsibilities

• Perform comprehensive patient evaluations, including history and physical exams.
• Diagnose and manage acute and chronic conditions.
• Order and interpret labs, imaging, and other diagnostics.
• Prescribe medications and therapies within scope.
• Develop treatment plans with Medical Director.
• Monitor progress and adjust care plans as needed.
• Initiate emergency procedures when indicated.
• Provide preventive care and health education.
• Maintain accurate documentation per standards.
• Collaborate with physicians and healthcare team for coordinated care.
