Aspiring healthcare team in the Greater Sacramento area seeks a full-time Nurse Practitioner or Physician Assistant to deliver comprehensive primary care in a community health setting. You will collaborate with physicians and multidisciplinary teams to provide high-quality, accessible care to underserved populations.

The role requires active CA NP license, NP board certification, and DEA; BLS is also required.

Formación

• Active California RN and NP license
• Current NP board certification
• Current DEA registration
• Current BLS certification
• 1–2 years clinical experience, preferably in community health or underserved populations
• Strong commitment to serving low-income, homeless, or medically underserved communities
• Comfortable handling three patients per hour
• EPIC experience
• In-basket management competency
• Bilingual English and Spanish strongly preferred

Responsabilidades

• Perform comprehensive patient evaluations including history and physical examinations
• Diagnose and manage acute and chronic medical conditions
• Order and interpret laboratory tests, imaging, and other diagnostics
• Prescribe medications and interventions within scope of practice
• Develop and implement treatment plans with Medical Director
• Monitor patient progress and modify care plans as needed
• Initiate emergency procedures when clinically indicated
• Provide preventive care, health education, and counseling
• Maintain accurate documentation per agency standards
• Collaborate with physicians and health care teams for coordinated care
