Job summary

LRSolutions, LLC is seeking a full-time Nurse Practitioner to provide comprehensive primary care in the Greater Sacramento area, CA. This role supports underserved populations, works with a multidisciplinary team, and offers relocation assistance. Mon-Fri 8-5 or a 4-day shift is available.

Qualified candidates will hold an active California NP license, NP board certification, DEA, and BLS, with 1–2 years of experience preferred and EPIC proficiency. Bilingual English/Spanish is highly valued.

Qualifications

Active California RN and NP licenses.
Current NP board certification.
Current DEA certification and BLS.
1–2 years clinical experience, ideally in community health.
EPIC experience.
In-basket management competency.
Bilingual in English and Spanish strongly preferred.

Responsibilities

Perform comprehensive patient evaluations including history and physical examinations.
Diagnose and manage acute and chronic medical conditions.
Order and interpret laboratory tests, imaging, and other diagnostics.
Prescribe medications and therapeutic interventions within scope of practice.
Develop and implement treatment plans with Medical Director.
Monitor patient progress and modify care plans as appropriate.
Initiate emergency procedures when clinically indicated.
Provide preventive care, health education, and counseling services.
Maintain accurate and timely documentation.
Collaborate with physicians and multidisciplinary teams for coordinated care.
