Job summary

Management Performance Associates in the Greater Sacramento area seeks a full-time Nurse Practitioner or Physician Assistant to deliver comprehensive primary care within a community health center. Role includes collaboration with physicians and care teams to serve diverse populations.

Responsibilities cover patient evaluations, diagnostic testing, treatment planning, and preventive care, with relocation assistance available and a Monday–Friday schedule. EPIC experience preferred.

Qualifications

• Active California RN and Nurse Practitioner license.
• Current Nurse Practitioner board certification.
• Current DEA certification.
• Current BLS certification.
• 1-2 years of clinical experience preferred, especially in community health or underserved populations.

Responsibilities

• Perform comprehensive patient evaluations, including history and physical examinations.
• Diagnose and manage acute and chronic medical conditions.
• Order and interpret laboratory tests, imaging, and other diagnostics.
• Prescribe medications and therapeutic interventions within scope of practice.
• Develop and implement treatment plans in collaboration with the Medical Director.
• Monitor patient progress and modify care plans as appropriate.
• Initiate emergency procedures when clinically indicated.
• Provide preventive care, health education, and counseling services.
• Maintain accurate and timely documentation in accordance with agency and regulatory standards.
• Collaborate with physicians and other healthcare professionals to ensure coordinated, high-quality care.
