Job summary
Top Echelon Software LLC is seeking a full-time Nurse Practitioner to provide primary care in a community health setting in the Greater Sacramento area. You will evaluate patients, manage acute/chronic conditions, and collaborate with multidisciplinary teams.
Requirements include CA NP license, NP board certification, DEA and BLS certifications, and 1–2 years in community health. Bilingual candidates are strongly preferred.
Qualifications
- CA NP license and NP board certification required.
- DEA and BLS certifications required.
- 1–2 years clinical experience, preferably in community health.
- Experience serving underserved populations highly valued.
- Bilingual English plus Spanish or other languages preferred.
Responsibilities
- Perform comprehensive patient evaluations including history and physicals.
- Diagnose and manage acute and chronic conditions.
- Order/interpret labs and imaging; adjust treatment plans.
- Prescribe medications within scope of practice.
- Collaborate with Medical Director and care teams.
- Provide preventive care and health education.
- Maintain accurate documentation per standards.
- Support high-quality, coordinated patient care.
