Job summary

Matrix Medical Network hires nurse practitioners to perform comprehensive in-home health assessments for adults and geriatric patients, with visits lasting up to an hour. Providers review medical history, medications, and risk factors to gain insight beyond routine visits.

Qualifications

• Master’s degree in nursing and NP licensure.
• Board certification in an adult-gerontology or family NP specialty.
• Current BLS/ACLS or CPR certification.
• Valid state NP license and prescriptive authority as required.

Responsibilities

• Conduct adult/geriatric assessments with medical history, diagnosis and treatment documentation.
• Administer and order point-of-care and laboratory testing as needed.
• Collaborate with physicians, case managers, social workers, and family members.
• Coordinate with primary care physicians on patient education and follow-up.
