Job summary

Qualifications
- Master’s degree or commensurate experience and NP licensure.
- Current NP licensure in state of practice with prescriptive authority.
- AANP/AACN/ANCC board certification in relevant adult-gerontology or family specialties.
- Active BLS/ACLS certification.
- 1 year NP experience preferred; new grads welcome.
- Valid state driver’s license and automobile insurance for residence state.
Responsibilities
- Conduct adult/geriatric health assessments including history and exam.
- Administer and order point-of-care and lab-based testing as needed.
- Collaborate with physicians, case managers, social workers, and caregivers.
- Educate patients and coordinate follow-up with PCPs.
- Provide services at member homes or Skilled Nursing Facilities.
