Job summary

Matrix Medical Network seeks Nurse Practitioners to perform comprehensive in-home health assessments for adults and geriatric patients. Visits can last up to one hour and cover medical history, medications, social factors, and care gaps, with documentation per company protocols.

Providers administer point-of-care tests when needed and collaborate with PCPs, case managers, and staff. Roles offer flexible scheduling, travel between homes and facilities, and opportunities to transition to full-time

Qualifications

• Master's Degree or commensurate NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in one of the specified NP specialties (AANP/AACN/ANCC).

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, referrals, follow-up and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members and other care teams.
• Coordinate with PCP for patient education and follow-up as necessary.
• Provide services at the member's residence (homes or Skilled Nursing Facilities).
