Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform Comprehensive Health Assessments in members' homes across the United States. Each visit lasts up to an hour and includes review of health, history, medications, and social factors to close care gaps.

You will work with physicians, case managers, and other staff to deliver personalized care, with visits scheduled based on your availability, including weekends.

Qualifications

• Master’s degree or equivalent clinical experience with NP licensure.
• Current NP licensure in practice state with prescriptive authority.
• Board certification in adult or related NP specialty.
• Valid driver’s license and automobile insurance.

Responsibilities

• Conduct adult/ge***riatric assessments including medical history, diagnosis and treatment, education, referrals and documentation.
• Administer and order point of care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP on education and follow-up.
• Provide services at the member’s residence (home or SNF).
