Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform comprehensive in-home health assessments. Each visit can last up to one hour and focuses on current health, medical history, medication adherence, social environment, and risks to identify care gaps.

Providers conduct geriatric assessments, may administer point-of-care tests, and collaborate with physicians and care team to improve outcomes. We offer flexibility, a wide scheduling window, and competitive compensation.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers as appropriate.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (home or Skilled Nursing Facility).
