Job summary

Matrix Medical Network seeks Nurse Practitioners to perform Comprehensive Health Assessments in members' homes or Skilled Nursing Facilities. Each visit spans up to one hour, reviewing health history, medications, social factors, and risks to close care gaps.

Providers order and perform point-of-care tests, work with physicians and care teams, and educate patients on care plans. Flexible scheduling, competitive pay based on visits, and FT opportunities with sign-on bonuses are offered.

Qualifications

• Must hold NP licensure in practicing state
• Board certification in Adult/Family/Gerontology or acute care NP
• BLS/ACLS or CPR certification
• 1 year NP experience preferred; new grads welcome
• Valid state driver’s license and auto insurance suitable for residence

Responsibilities

• Conduct adult/geriatric assessments, documenting medical history, diagnoses, treatment, education, referrals and follow-up
• Administer and order point-of-care and/or lab-based testing as needed
• Collaborate with physicians, case managers, social workers, family caregivers and ancillary staff
• Coordinate with PCPs on patient education and follow-up
• Provide services at member homes or SNFs as required and per Matrix guidelines
