Job summary

Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. The role focuses on education, gap closure, and collaboration with primary care providers to improve health outcomes.

The position offers flexible scheduling, no on-call, weekends, or holidays, and requires active NP/PA certification or NCCPA certification, state licensure, and a valid driver's license for home visits.

Qualifications

• Active ANCC or AANP certification for NPs or NCCPA certification for PAs
• State licensure in the practicing state (unrestricted)
• Active driver's license and reliable transportation for home visits
• Experience with electronic medical records (EMR) and health tech
• Strong communication with geriatric or Medicare populations
• Ability to navigate varied home environments during assessments

Responsibilities

• Conduct comprehensive in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses for care management and treatment planning and communicate findings to PCPs.
• Identify urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed
