This field-based, non-prescribing role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes. Flexible hours 20–40 per week with sign-on bonus and benefits.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted license in the practicing state, without disciplinary actions.
• Access to reliable transportation for home visits.

Responsibilities

• Conduct comprehensive in-home assessments including history, vitals, exam.
• Perform screenings and point-of-care testing as appropriate.
• Document diagnoses and coordinate with PCPs to close care gaps.
• Educate patients on diseases, medications and compliance.
• Identify urgent cases and intervene when needed.
