Job summary

Optum Home & Community Care, part of the Optum family, is hiring a HouseCalls Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other members. You’ll work in an interdisciplinary team, staying flexible with 8am-8pm scheduling across seven days and no on-call requirements.

Requirements include NP or PA certifications, active state licensure, and a driver's license with reliable transportation for home visits.

Qualifications

• Active ANCC or AANP certification for NP roles or NCCPA for PA roles.
• Active, unrestricted state licensure with ability to practice.
• Reliable transportation and valid driver’s license for home visits.
• Excellent communication and ability to navigate diverse home environments.

Responsibilities

• Conduct comprehensive in-home assessments including medical history, meds, vitals and exams.
• Document diagnoses and create care plans for chronic or preventive care.
• Communicate findings to primary care providers to close care gaps.
• Educate patients on disease processes, medications and adherence.
• Address social determinants of health and arrange referrals as needed.
