Job summary

Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to perform annual in-home health assessments for Medicare Advantage and other plan members. This non-prescribing, field-based role emphasizes education, gap closure, and collaboration with primary care providers.

The position offers flexible scheduling (8am–8pm, 7 days), a sign-on bonus, competitive compensation, and a supportive environment focused on patient-centered care and work-life balance.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the practicing state.
• Active and unrestricted driver's license with reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.

Responsibilities

• Conduct in-home assessments including medical history review, vital signs, and exams.
• Document diagnoses for care management and planning.
• Communicate findings to patients' PCPs to close care gaps.
• Identify urgent situations and intervene when needed.
• Educate patients on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
