Job summary
UnitedHealth Group's Optum Home & Community Care is seeking an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve health outcomes.
The position supports flexible scheduling, no on-call, and travel to patient's homes within a defined service area.
Qualifications
- For NPs: Active ANCC or AANP Family/Adult/Geriatric Primary Care certification
- For PAs: Current NCCPA certification and state licensure
- Active, unrestricted licensure in the practicing state and ability to obtain by start date
- Active and unrestricted driver's license with transportation to complete home visits
Responsibilities
- Conduct comprehensive in-home assessments for Medicare Advantage and other plans
- Document diagnoses for care management and treatment planning
- Communicate findings to PCPs to close gaps in care
- Identify urgent situations and intervene appropriately
- Educate members on diseases, medications, and compliance
- Address social determinants of health and provide referrals
