Job summary

Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage members, emphasizing education, gap closure, and PCP collaboration.

Active NP/PA license with national certification is required; ability to travel up to 85% and meet physical requirements are essential. A $20,000 sign-on bonus is offered for external candidates, with flexible scheduling and no on-call or weekend work.

Qualifications

• Active NP or PA license by start date without encumbrances.
• National certification in Family, Geriatrics, Adult or related field (or NCCPA for PAs).
• Ability to move a 30 lb bag, navigate stairs, and perform physical assessments.
• Reliable transportation to complete home visit assessments.

Responsibilities

• Conduct in-home assessments including medical history, vitals, and physical exam.
• Perform screenings and coordinate with PCPs to close care gaps.
• Identify urgent situations and intervene appropriately.
• Educate members on disease processes and medication adherence.
• Travel to member homes up to 85%.
