Job summary

Optum HouseCalls is seeking a qualified Advanced Practice Clinician (Nurse Practitioner) to perform in-home health assessments for Medicare Advantage members. This non-prescribing, field-based role focuses on education, gap closure, and collaboration with primary care providers.

You will conduct comprehensive assessments, document diagnoses, and develop care plans while addressing social determinants of health. Strong communication with PCPs and ability to travel to patient homes are required.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted licensure in the practicing state without disciplinary actions.
• Active and unrestricted driver's license with access to reliable transportation for home visits.
• Ability to navigate varied home environments and perform assessments.
• If applicable, eligibility and approval steps for independent NP/PA practice must be initiated within timelines.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other plan members.
• Document diagnoses and care plans; identify gaps in care and communicate with PCPs.
• Educate patients on disease processes, medications, and self-management.
• Address social determinants of health and coordinate referrals as needed.
• Recognize urgent situations and intervene appropriately.
