Job summary

Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to conduct in-home assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.

Flexible scheduling allows 20-40 hours per week with 8am-8pm coverage, no on-call, weekends, or holidays. A $10,000 sign-on bonus is available to eligible candidates and a comprehensive benefits package is offered.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active/unrestricted licensure to practice in the state with start date requirements
• Driver's license and reliable transportation for home visits
• Ability to navigate varied home environments for assessments

Responsibilities

• Conduct comprehensive in-home assessments including medical history, vitals, and physical exam
• Document diagnoses for care management and treatment planning
• Communicate findings to PCPs to address gaps in care
• Recognize urgent situations and intervene as needed
• Educate members on diseases, medications and adherence
• Address social determinants of health and provide referrals as needed
