The position offers flexibility in scheduling (8 AM–8 PM, 7 days/week) with no on-call requirements, weekends, or holidays, and potential additional pay for language skills. A strong driver’s license is required.

Qualifications

• Active NP or PA certification as required by the role.
• State licensure active and unrestricted.
• Must have a valid driver’s license and reliable transportation.
• Experience performing assessments in diverse home environments.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other plans.
• Collaborate with primary care providers to close care gaps.
• Educate patients and caregivers to improve health outcomes.
• Maintain autonomy in a field-based, non-prescribing role.
