Qualifications

• Minimum 6-months Nurse Practitioner experience in primary care/internal medicine.
• Value-based care experience preferred.
• Must be comfortable providing care in patients' homes.

Responsibilities

• Develop and implement clinical plans of care for adult patients facing chronic conditions.
• Gather patient history and diagnostics in home or clinic setting.
• Collaborate with interdisciplinary team for patient care.
