Job summary
Upperline Health Inc in Orange City, FL is seeking a Nurse Practitioner to provide clinic-based care in collaboration with the local physician team. You will develop complex plans of care for adults with chronic conditions and coordinate with an interdisciplinary team.
This role emphasizes compassionate, home- and clinic-based care within a value-based care model. A weekday schedule, ongoing education, and opportunities for growth are offered along with a competitive, bonus-eligible compensation
Qualifications
- Master’s degree in nursing is required.
- Board Certification as Adult, Adult-Geriatric or Family NP required.
- At least 6 months NP experience in primary care/internal medicine.
- DEA registration or eligibility.
- Basic Life Support (BLS) certification required.
- Experience with Home-based care or value-based care preferred.
- Comfortable providing care in patients' homes.
Responsibilities
- Develop and implement clinical plans of care for adults with chronic conditions.
- Gather history, exam, and diagnostics in home or clinic.
- Collaborate with interdisciplinary team to meet physical and psychosocial needs.
- Educate patients and families on medications, diet, and prevention.
- Maintain medical records in line with state regulations.
- Participate in weekly team meetings and continued education.
- Prescribe medications as permitted by the state Board of Nursing.
