Job summary

Upperline Health in Citrus County, FL is seeking a Nurse Practitioner to deliver care in a clinic and home-based settings, collaborating with a multidisciplinary team. You will develop complex care plans, perform exams, and educate patients on medications, diet, and crisis prevention as part of a value-based care model.

The ideal candidate holds a Master’s in nursing, NP certification, and an active RN/NP license with DEA eligibility.

Qualifications

• Master’s degree in nursing required.
• Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner required.
• Current State RN & Nurse Practitioner license required.
• Minimum 6 months NP experience in primary care/internal medicine.
• Value-based care experience preferred.
• DEA registration or eligibility.
• BLS certification required.
• Comfortable providing care in patients' homes.
• Experience with EMR (AthenaHealth preferred).
• Proficiency with Microsoft Office.

Responsibilities

• Develop and implement clinical plans of care for adult patients with chronic conditions.
• Gather history, physical exam, and diagnostics in home or clinic.
• Collaborate with interdisciplinary team to meet physical and psychosocial needs.
• Educate patients and families about medications, diet, and crisis prevention.
• Coordinate care with other providers and maintain medical records.
• Prescribe medications as permitted by state Board of Nursing.
