Job summary
Upperline Health Inc. is seeking a Nurse Practitioner to provide clinic-based care in our Palm Coast, FL clinic.
APPs develop complex care plans with the physician team, provide diagnostics and treatments in clinic, home, or telehealth, and educate patients on medications, diet, and prevention. Ideal candidates hold a Master’s in nursing, board certification (Adult/Adult‑Geriatric/Family NP), DEA registration, and BLS; 6 months of primary care experience is preferred.
Qualifications
- Master’s degree in nursing required.
- Board Certification as an Adult, Adult‑Geriatric or Family Nurse Practitioner required.
- DEA registration or eligibility.
- Basic Life Support (BLS) certification required.
Responsibilities
- Develop and implement plans of care for adults with chronic conditions in clinic or home settings.
- Gather patient history, perform examinations, and order diagnostics.
- Collaborate with the interdisciplinary team to coordinate comprehensive care.
- Educate patients and families on medications, lifestyle, and crisis prevention.
- Prescribe medications as permitted by state boards.
