Qualifications

• 6+ months of Nurse Practitioner experience in primary care or internal medicine.
• Comfortable providing care in patients' homes.
• Basic Life Support certification is required.

Responsibilities

• Develop and implement clinical plans of care for adult patients facing chronic conditions.
• Gather patient history, physical exam, and diagnostics.
• Communicate with Upperline’s local physician to review care plans.
