Job summary

Upperline Health in Orange City, FL seeks a Nurse Practitioner to provide clinic-based care in collaboration with a physician team. You will develop complex plans of care for adults with chronic conditions and deliver diagnostics and treatments in clinic or home settings, with telehealth as needed.

Ideal candidates hold a Master’s in nursing, NP board certification, and FL RN/NP licenses, with DEA eligibility and BLS.

Qualifications

• Master’s degree in nursing required.
• Board Certification as an Adult, Adult-Geriatric or Family NP required.
• Current State RN & NP license required.
• DEA registration or eligibility.
• BLS certification is required.
• Experience in home-based care or value-based care is preferred.

Responsibilities

• Develop and implement clinical plans of care for adults with chronic conditions.
• Gather patient history, exams, and diagnostics in home or clinic.
• Develop and implement treatment plans aligned with patient goals.
• Collaborate with interdisciplinary team to meet physical and psychosocial needs.
• Provide compassionate care for complex patients and families.
• Educate patients on medications, diet, adherence, and crisis prevention.
• Coordinate with community providers to ensure optimal care.
• Review records and care plans with local physician in a timely manner.
• Maintain medical records per regulations and policy.
• Participate in team meetings and continuing education.
• Prescribe medications as permitted by the Board of Nursing.
• Other related duties as needed.
